AZ interview with dermatologist Sophie Müller-Wiefel

What is circular hair loss? who causes does alopecia areata have? What does the treatment look like? And how good are baricitinib tablets? AZ wanted to know from specialist Sophie Müller-Wiefel. The 39-year-old man works in dermatology at Sendlinger Tor.

AZ: Dear Mrs. Müller-Wiefel, where does hair most often fall out?
SOPHIE MUELLER-WIEFEL: The whole body may be affected. Eyelashes and eyebrows can also fall out. If rounded hairless areas develop on the body, the body should be examined for alopecia areata. The upper and back of the head are most often affected.

How many Germans suffer from circular hair loss?
It is estimated that one to two percent of the population suffers from it, ie about 1.5 million Germans. By the way, women and men are equally affected.

What are the first signs and symptoms?
It’s classic hair loss and not necessarily itching. Bald spots often appear after a shower or at the hairdresser’s.

What advice do you have for affected patients?
Don’t panic and google! About 50 percent of hair grows within a year without treatment.

What are the causes?
Alopecia areata is a form of autoimmune disease. The immune system defends itself against the body itself – specifically against the hair follicles. Why this is the case has not yet been definitively investigated. Genetics plays a major role, but other factors, such as environmental influences or (emotional) stress, can also cause irregular hair loss.

What can you do? Cortisone cream reduces inflammation

What does the treatment look like?
Alopecia areata often occurs in phases between years. The disease is chronic. Circular hair loss cannot be cured, but it can be treated. Thyroid levels as well as zinc and vitamin D levels should be checked. You can cream the affected areas with cortisone to reduce inflammation.

Active substance baricitinib with side effects

People have also been reading about baricitinib tablets for a few weeks now.
The drug with the active substance baricitinib has already been approved in the USA. Studies have shown that hair grows back when swallowed. However, there are side effects: the immune system declines, viruses can be reactivated; Herpes and shingles can therefore break out more often. If a patient has severe bald spots, the benefits and risks should be considered. Regular blood tests are necessary.
